SMAS facelift in Germany

Updated

Two completely different things are sold as “SMAS lifting”: a real operation under anaesthesia that lifts the layer beneath the facial skin, and a device treatment with no incisions. Anyone looking for a SMAS facelift in Germany often finds the other one instead.

This page covers the surgical SMAS facelift: what the SMAS is, why surgeons lift it, who the operation suits, how it is done and what recovery looks like. For other operations, see the section on plastic surgery in Germany.

What the SMAS is and why it is lifted

The SMAS is a firm layer under the facial skin, lifted because it is what descends with age, together with the fat pads. It is a thin muscle with its covering sheet, connected to the muscles of the neck. The skin merely follows and only looks like the culprit.

Pulling the skin alone gives a tight, wind-blown look, and the face soon slips back because skin stretches again. So the surgeon works on the layer: folds and stitches it (plication), removes a strip (SMASectomy), or frees it through deeper dissection and moves it (deep plane, high SMAS).

The operation and device «SMAS lifting» are not the same

Surgery moves the descended layer; treatments without incisions only firm up tissue. Without surgery, the same name covers focused ultrasound (HIFU), which heats tissue at depth, radiofrequency devices and threads. They act on firmness and tone but do not move a dropped layer, so results are gentler and shorter-lived.

With small changes and at a younger age, device treatments are a reasonable step to discuss. With pronounced sagging and excess skin they do not replace surgery, as a review of non-surgical tightening notes, and an honest doctor says so.

Be wary of adverts promising “a lift without surgery with a surgical effect”, silent about risks, or with photos in different light and different poses. In Germany surgery and devices each have their own indications, and the choice is made at a consultation, not from a price list. The types of lift are compared on the facelift in Germany page.

Who the operation suits and what is assessed at the consultation

The operation suits people whose facial and neck tissues have dropped: sagging cheeks, deepened folds beside the mouth, a lost jawline, a slack neck. The surgeon assesses not only the skin but also tissue volume, the neck and facial movement, and may propose something other than what the patient came for.

Smoking, blood pressure, diabetes, blood-thinning medicines, hormones and earlier operations and injections in the face are always discussed: all of them affect healing and the plan.

To set expectations, the surgeon takes photographs, marks the face and explains what is achievable for it, answering plainly: a lift returns tissue to its position but does not change skin quality or remove fine lines, which need other methods. Blepharoplasty, a brow lift or neck work are often discussed for the same session.

How the operation is done

The surgeon lifts the skin, works on the SMAS, then redrapes the skin without tension and trims the surplus. Incisions follow natural lines in front of the ear, behind it and into the hair, so scars hide in folds and the hairline.

General anaesthesia or sedation with local anaesthetic is used, as the anaesthetist decides. Drains are placed if needed, a soft pressure dressing is applied and the patient is observed afterwards. The clinic states how long the operation and the stay take.

In Germany, plastic and aesthetic surgeons perform it. At Villa Bella Clinic in Munich, aesthetic facial surgery is done by its head, Dr Ludger Meyer-Dobbelstein; facial lifting is also among the fields of Dr Sebastian Pretzsch there. At Diakonie Hospital Stuttgart, Prof. Frank Werdin heads the department of aesthetic and plastic surgery and hand surgery.

Recovery and risks

Recovery needs patience: the final appearance takes months. The first days and weeks bring swelling, bruising, numbness and tightness in front of the ears. The patient wears the dressing, is careful with facial movement, sleeps with the head raised and attends wound checks and stitch removal; swelling settles gradually.

Risks discussed before surgery include haematoma, infection, poor skin healing especially in smokers, temporary weakness of facial movement if a branch of the facial nerve is irritated (rarely a lasting one), changed sensation, a visible or widened scar and asymmetry. Some need a further operation, which is also discussed in advance. Germany’s society of plastic surgeons, DGPRÄC, explains them in its patient information.

Much depends on the patient: giving up smoking, sun protection for the scars, care with physical activity, patience with swelling and keeping appointments. Honestly, the operation turns the appearance back but does not stop ageing, and years later the conversation may return.

Frequently asked questions

How does a SMAS facelift differ from a skin-only lift?

A SMAS facelift lifts the layer beneath the skin, not the skin itself. That layer sags with age along with the fat pads, so the surgeon repositions it and lays the skin over it without tension. A skin-only pull looks tight and soon slips back as skin stretches again.

Can ultrasound or threads replace a SMAS facelift?

Not when sagging is pronounced and there is excess skin. Focused ultrasound, radiofrequency and threads firm up tissue but do not move a dropped layer, so the result is gentler and shorter-lived. With small changes at a younger age they are a reasonable step to discuss.

Does a SMAS facelift remove fine lines?

No: a lift returns tissue to its position but does not change skin quality or remove fine lines, which need other methods. Nor does it stop ageing: the appearance is turned back, but the face keeps changing. The consultation shows what is achievable for a particular face.

How AlenMed arranges the operation

AlenMed sends the clinic photographs of the face and neck, the list of medicines and details of earlier operations and injections. The surgeon says whether surgery is an option or something else makes more sense, whether an anaesthetic assessment is needed and what to examine on site. Next come a written estimate, a visa invitation, consultation and surgery back to back, and an interpreter at appointments. If the neck is included, see neck lift cost in Germany.

Plan enough days in Germany for the check before flying home, a companion, accommodation near the clinic and a doctor at home to look at the wounds and scars.

This material is for information only and is not an offer; a doctor decides on the indication and extent of surgery in person.
