A brow lift raises a dropped brow, yet what brings people here is rarely the forehead. It is a face that looks tired or stern in photos, brows lower than before, upper lids that seem to hang though the lid itself has barely changed. Often the brow is the cause, the lid only the symptom, and the two need completely different operations.
Below: what German surgeons mean by a brow lift, how the technique is chosen, the first weeks, and what a patient from abroad must plan for a brow lift in Germany. The wider field: plastic surgery in Germany.
Brow lift or eyelid surgery: the difference that decides everything
A mirror test tells a dropped brow from excess lid skin. Lift the outer end of the brow slightly with a fingertip: if the heaviness goes and the lid looks open again, the brow has dropped, not the lid; if nothing changes, the excess skin is on the lid.
Medically, this matters: removing upper lid skin under a dropped brow can pull the brow lower still and, in extreme cases, leave the eye harder to close, with dryness and irritation. It is a common reason for disappointment after eyelid surgery.
Often both are true; then the order is fixed: the brow is lifted first, and only then is it decided how much lid skin, if any, is still in the way. The lid operation itself: eyelid surgery in Germany.
The techniques used in Germany
For a whole-brow lift, the endoscopic technique is used most often: through several short incisions behind the hairline the surgeon works with a camera, releases the tissue and fixes it higher; scars stay in the hair.
The temporal (lateral) lift raises only the brow tail through two small temple incisions — what most patients need: a lifted tail, the inner brow untouched. The coronal lift (a long scalp incision) is kept for heavy brows with deep forehead folds and moves the hairline back, limiting its use. Men with a high or receding hairline, unsuited to a hair incision, get one directly above the brow; small descents can be lifted via the upper lid.
The choice depends on hairline height, forehead length, hair density, skin thickness, whether the whole brow or only its tail needs lifting, and keeping the face recognisable, not surprised. The middle and lower face belong to the neighbouring facelift in Germany and SMAS facelift in Germany, sometimes combined with a brow lift.
What a serious consultation decides
A serious consultation decides whether and how to operate; examination comes before any figure. The surgeon takes photos at rest and in motion, measures brow height against the bony rim, checks lid crease and function, and asks about dry eyes, thyroid disease, previous eyelid surgery and recent botulinum injections, which alter the picture for months.
You leave with a written plan: technique, incision lines, anaesthesia, number of nights and follow-up visits included in the price. German practice also allows time for reflection before an aesthetic operation, and a second opinion is normal. On the estimate: plastic surgery prices in Germany.
Then language and distance: photos and questions can be exchanged before the trip, the consultation held in English or through an interpreter; now establish who answers a question three months later, when you are home and one brow sits differently.
The operation and the first weeks
A brow lift usually takes one to one and a half hours under local anaesthesia with sedation or general anaesthesia, depending on technique and combined procedures. A night of observation is sometimes needed, not always.
First days: swollen forehead and lids, bruising that may spread to the cheeks, tightness, and numbness behind the incision that fades over weeks to months, a normal effect of the approach, not a complication. Keep the head high, use cooling; stitches or clips come out when the surgeon says.
Travellers need a timetable: days in Germany before and after surgery, the first follow-up, when flying is allowed, when sport, sauna, hair colouring and heavy lifting can resume, and how long the scar needs sun protection. The final brow shape is judged after months, not from the first clinic photo.
Risks and what can be corrected
The honest list: asymmetry between the sides, a brow raised too high with a permanently startled look, altered scalp sensation, hair thinning along the incision, a visible scar in a thin hairline, and the descent slowly returning over years, since the operation moves tissue but does not stop ageing.
Small asymmetries can be corrected by a minor procedure once the swelling has fully settled. A revision has its own cost and waiting time, so ask before the first operation who pays for it and on what terms. On preventing surgical harm: the WHO fact sheet on patient safety.
Frequently asked questions
How long does a brow lift last?
For years, but not forever: the operation moves tissue while ageing goes on, so over time the brow slowly descends again. The final shape is judged after several months, once the swelling has gone, and nobody can name an exact period in advance.
Where are the scars after a brow lift?
Along the incisions: short scars behind the hairline (endoscopic lift), small ones in the temple hair (temporal), a long one across the scalp (coronal), one above the brow (direct) or in the lid crease (lid route). Thin hair shows them more; keep them out of the sun for several months.
Can a brow lift be combined with eyelid surgery?
Yes, in a fixed order: the brow is lifted before or during the same operation, and only then is the remaining lid skin assessed. Sometimes the surgeon stages the procedures so the eye still closes properly. Skipping the brow is risky: it can drop lower, and the result disappoints.
How AlenMed arranges a brow lift in Germany
AlenMed sends photos, measurements and relevant history to surgeons with the German specialist title in plastic, reconstructive and aesthetic surgery who perform this operation regularly; each names the technique he finds suitable and what he would advise against before any figure is named. Then: the written plan, the estimate, the visa invitation, an interpreter at consultation and discharge, and accommodation until the follow-up. Where they work: plastic surgery clinics in Germany.
Plan your days in Germany, the visit where the result is reviewed, and how a later question reaches the same surgeon from home.
