Liposuction in Germany is about shape, not weight: it removes local fat that often resists diet and exercise. A German plastic surgeon will say so at the first consultation, as does the surgeons’ society DGPRÄC.
Below: who it suits, the techniques used in Germany, surgery and recovery, and how it differs from lipoedema surgery. For the wider field, see plastic surgery in Germany.
What liposuction can and cannot do
The aim is contour, not kilograms. The surgeon shapes the area rather than emptying it, and volume per session is limited: the more fat comes out, the more fluid and blood are lost, sometimes needing infusions or a transfusion. Safety sets that limit.
It works best on clearly defined deposits: flanks, thighs, arms, back, under the chin. On the abdomen, the cannula reaches fat under the skin but not visceral fat around the organs. Stretched skin and muscles separated by pregnancy or years of excess weight need more than liposuction: a tummy tuck in Germany.
Liposuction does not treat obesity, remove cellulite or tighten skin. Cellulite usually stays the same, and only elastic skin settles onto the new contour. People with general excess weight are advised to lose weight first.
Techniques: tumescent, water-jet, ultrasound, laser
All techniques share one principle: fat is suctioned out through fine cannulas via small incisions. The area is often first infiltrated with local anaesthetic and vessel-narrowing adrenaline, which eases suction and reduces bruising and blood loss; with a large volume the tissue swells, hence “tumescent”.
Water-jet-assisted liposuction (WAL) loosens fat with a fan-shaped jet; the DGPRÄC calls it the most widely used method besides tumescent liposuction. Power-assisted liposuction (PAL) uses a vibrating cannula for dense fat, while ultrasound (UAL) is rarely used, mostly on firm tissue such as the back. A 2026 systematic review found all techniques removed fat comparably, so area, tissue and experience decide, not marketing. Laser: see laser liposuction in Germany.
Lipofilling transfers the patient’s own liposuctioned fat to where volume is lacking, such as the face or breast. It is planned separately: harvesting affects fat quality, and an unpredictable share of the graft is absorbed, so a second session is sometimes needed.
Preparation and the day of surgery
At the consultation the surgeon examines and photographs the areas and discusses expectations. Tests are ordered, painkillers containing acetylsalicylic acid stop 14 days before surgery, and nicotine and alcohol are kept minimal. German law requires a doctor to explain risks and alternatives in person and in good time before consent is signed.
Local anaesthesia, already in the tumescent solution, may suffice for the hips and legs; larger operations need general anaesthesia, some techniques general anaesthesia or deep sedation. Most patients go home the same day, after a larger operation one to two days later.
The surgeon marks the areas and checks them with the patient standing, as contours shift lying down. Incisions of a few millimetres to a centimetre are hidden in creases or the navel. The fat removed is measured so both sides match; a drain is placed if needed. Depending on the areas, surgery takes 20 minutes to two hours, tumescent liposuction up to five hours.
Recovery: compression, swelling, when the result shows
The compression garment goes on in theatre and is worn day and night for six weeks. It is treatment, not an accessory: it stops the spaces left by the fat filling with fluid and blood, which can cause lumps.
Bruising and mild pain usually fade within days to weeks, swelling within three months, numbness within several weeks. Walking helps against swelling and thrombosis. Sport, massage and baths wait as long as the surgeon says; manual lymphatic drainage is often prescribed, and a 2026 review found it moderately reduces swelling.
The final shape can usually be judged after six to nine months, as healing outlasts the swelling. Removed fat rarely returns, but remaining fat cells grow with weight gain. In a 2012 randomised trial, visceral fat rose by 10% after abdominal liposuction in women who did not exercise, but not in those who trained. The result therefore depends on stable weight and activity. On costs, see plastic surgery prices in Germany.
When the “fat” is really lipoedema
A German doctor suspects disease, not a cosmetic issue, when fat sits symmetrically on legs or arms that are clearly fuller than the trunk, the feet and hands are spared and the tissue hurts when pressed or touched. Easy bruising is often reported but not decisive. This is lipoedema, a separate condition: see lipoedema surgery in Germany.
Surgery is not planned without a diagnosis: painless excess fat on the legs is lipohypertrophy, not lipoedema. Under the German 2024 guideline, the main reason to operate is pain persisting despite conservative treatment. Coexisting obesity is treated first, a lymph-sparing technique is used, often over several sessions, and decongestive therapy follows straight away.
Frequently asked questions
How much fat can one liposuction remove?
As much as is safe; there is no single figure. The surgeon judges by touch which fat to remove and which to leave for an even contour. Do not expect a big drop in weight: a 2008 review notes it falls only by the fat removed, about 3–5 kg.
How safe is liposuction?
Serious complications are rare but possible. A 2017 US analysis of 31,010 cosmetic liposuction procedures found a major complication rate of 0.7% for liposuction alone, counting emergency visits, hospital admissions and reoperations within 30 days. Risk rose with combined operations, age and body mass index.
Can liposuction be repeated?
Yes, even in a treated area, though it is rarely needed. Surgeons wait until the first operation has fully healed, which can take up to a year. Irregularities lasting several weeks are sometimes corrected with a small procedure; skin that shrinks less than expected may need tightening.
How AlenMed arranges liposuction in Germany
AlenMed reviews photographs of the areas remotely, matches a plastic surgery clinic and surgeon to the task and sends a written estimate before you travel. We arrange the visa invitation and an interpreter for the consultation and the explanatory talk, which German law requires to be understandable.
Stay at least until the check-up a week after surgery, when, according to the DGPRÄC, the stitches can come out. We agree the flight date with the surgeon and, once you are home, keep you in touch and arrange a further check-up if needed. Suitable departments: plastic surgery clinics in Germany.
