Plastic Surgery in Germany: Prices and What They Include

People looking up prices for plastic surgery in Germany usually want a price list, but a surgeon names a sum only after seeing the patient, because the price depends on what exactly has to be done and how.

This page is not a price list. It explains how the price of any cosmetic or reconstructive operation is built and where to find pages on individual procedures; the operations themselves are covered under plastic surgery in Germany. The only euro figure already on AlenMed’s prices page is for nasal surgery, including correction of the shape — from €4,000.

How a Plastic Surgeon in Germany Sets the Price

The surgeon’s fee follows the official German fee schedule for doctors, the GOÄ: each step of an operation has an item number, a base fee (points times the value of a point) and a multiplier. Cosmetic operations are included: services beyond medical necessity may be billed if the patient asked for them.

The multiplier for personal medical services ranges from 1.0 to 3.5 but as a rule does not exceed 2.3; going higher needs a reason, such as difficulty and time, given in writing on the invoice. A different fee level may be agreed in writing before the service (§ 2 GOÄ), and the agreement shows the item numbers, the multiplier and the amounts.

Private clinics and practice clinics add their own items: theatre, anaesthetist, overnight stay, materials and implants. Some combine everything into one total price, others list each item, so compare estimates by content, not by the bottom line.

Face, Breast, Body: Why Prices Differ Between Operations

Prices differ because operations involve different amounts of work. What counts is the length of the operation, the anaesthesia (local with sedation, a medicine that makes the patient drowsy, or general), going home the same day or staying overnight, and implants and materials.

On the face, examples are eyelid correction (see eyelid surgery costs) and facelifts. Breast surgery covers enlargement, lift and reduction, with pages on breast lift costs and breast reduction costs; on the body, a common example is liposuction (liposuction costs). A combined operation like a breast lift with implants is one estimate, not two prices added up: anaesthesia, theatre and stay are needed once.

The surgeon’s experience and the clinic’s format also change the sum. A university hospital department has anaesthesia and intensive care at hand, which matters for long and reconstructive operations; a small specialised practice clinic mainly does planned procedures, often without an overnight stay, and its costs are organised differently.

Cosmetic or Reconstructive: When the Reason Is Medical

The reason is medical when an operation is needed because of illness, injury or a congenital defect, or repairs the effects of earlier treatment: breast reconstruction after cancer, a drooping upper eyelid that limits vision, skin folds after major weight loss that keep getting inflamed. The doctor documents the reason in writing.

This matters for the invoice: in Germany a purely cosmetic operation without a medical reason is subject to VAT at 19%, while medical treatment by a doctor is exempt; the estimate should say which applies. The German Society of Plastic, Reconstructive and Aesthetic Surgeons (DGPRÄC) explains how to check a surgeon: the title Facharzt für Plastische und Ästhetische Chirurgie is protected, while any doctor may call themselves a «cosmetic surgeon».

What the Estimate Covers and What Comes on Top

A clinic estimate covers the doctors’ fees and the costs of the operation and stay; travel, accommodation, an interpreter and the visa come on top. Ask item by item about pre-operative consultation and tests, anaesthesia, the overnight stay, compression garments, follow-up visits and how a correction is handled.

On top of the estimate come flights, accommodation, an interpreter and the visa, which depend on where the patient travels from and for how long.

Time counts too: after most operations the patient stays near the clinic for follow-up checks and the surgeon decides when flying home is safe, so plan accommodation for that period and book the return flight only then.

Clinic prices as of October 2026: breast enlargement at Bonn University Hospital costs about €6,400 without an overnight stay and about €6,800 with one, VAT included; at Würzburg University Hospital’s eye clinic, upper eyelid surgery costs about €2,000 and all four eyelids about €4,000.

Is plastic surgery cheaper in Germany?

Not necessarily. The German price is made up of doctors’ fees under the official GOÄ schedule, anaesthesia, the theatre, the overnight stay and materials, plus VAT where there is no medical reason; flights, accommodation and an interpreter come on top. Compare what offers include, not their bottom lines: the surgeon’s specialist title, anaesthesia, follow-up visits and correction terms.

Can I get an exact price before I travel?

Not the exact one: the surgeon names the final sum after an in-person examination, as the plan for aesthetic surgery is made only at the consultation. From photos and medical information the surgeon can say in advance which procedure is worth discussing, and any different fee level is agreed in writing before the operation.
