Breast Augmentation in Germany

Updated

Breast augmentation is rarely a sudden decision: people think it over at length, then face dozens of offers reduced to price and cup size. Yet size comes far from first: the surgeon looks at chest width, tissue thickness, nipple position and whether the breast has dropped, which decide whether an implant suits at all, and which one.

Below: the German consultation, implant types and placement, how autologous fat transfer differs, recovery, the risks discussed beforehand, and payment.

What the consultation decides

The consultation decides whether an implant suits you and how large it may be. The surgeon measures breast base width, skin stretch and nipple-to-fold distance, and checks symmetry, scars and previous operations; the permissible volume follows from these measurements, not the reverse.

Then the aim: congenital asymmetry, volume lost after pregnancy, breastfeeding or weight loss, or restoring shape after breast surgery. A noticeably dropped breast needs a lift or a combination, as augmentation alone will not do (see Breast lift in Germany); the opposite task is Breast reduction in Germany.

Before consent, the surgeon should say openly what cannot be changed, how long the operation takes, what happens if the result disappoints, and why a smaller volume than requested may be proposed. German law requires timely explanation for a considered decision, in effect a reflection period before surgery, and you sign a written consent listing the risks, keeping a copy.

Implants, approaches and placement

Three choices are made: implant, position and incision. Breast implants in Germany usually contain thick, cohesive silicone gel that holds together; saline is now used only in exceptional cases. A teardrop, anatomical shape gives a smoother upper transition, a round one more upper fullness. Projection, how far the breast stands forward, and shell surface, smooth or textured, also vary. The choice follows the existing tissue.

Placement. In front of the muscle, the implant needs enough tissue cover, or its edges may show in slim women, but no muscle distorts it under load. Behind the muscle it is better covered and mammograms may show more breast tissue, yet a tightening chest muscle can shift it and reshape the breast. The dual-plane compromise covers only the upper part with muscle and moves less under load.

The incision: breast fold, areola edge or armpit. The fold gives the surgeon the best view and control, with the scar underneath; the areola edge hides the mark, though it stays on the breast; the armpit leaves none on the breast, but without an endoscope the surgeon sees less. Autologous fat transfer needs no implant: it looks natural, but the gain is modest, part of the fat is reabsorbed, two sessions are often needed, and it does not suit everyone.

The operation, recovery and risks

The operation usually takes one to two hours under general anaesthetic, followed by a short stay or one inpatient night, as the surgeon decides. Drains are usually placed, a compression bra goes on at once, and lifting is restricted for several weeks.

The breasts are swollen and tight for the first days. Desk work is usually possible within a few days; sport, sauna and flying wait for a period the surgeon sets. The final shape and softness take several months.

Risks discussed in advance: capsular contracture (the scar-tissue shell around the implant hardening), displacement, rupture, infection, changed nipple sensation, asymmetry and likely further surgery years later. Implants are not for life: the Federal Institute for Drugs and Medical Devices states that silicone breast implants are not designed to last a lifetime. Follow-up is needed, and the radiologist must know about the implant before and during a mammogram.

How this works in Germany

Breast augmentation in Germany is paid privately; the doctor’s fee follows the official GOÄ scale (see Cost of breast augmentation in Germany). Check for the protected title «specialist in plastic and aesthetic surgery», since a doctor without it may formally still operate in aesthetics. Breast implants in Germany go into the mandatory Implantateregister Deutschland: since 1 July 2024 the clinic reports every insertion, revision and removal, and you receive a copy of the data sent.

Partner departments as examples, not a ranking: at the Villa Bella practice clinic in Munich, Dr Nadja Zimmer handles female breast corrections (augmentation, reduction, lift), Dr Sebastian Pretzsch aesthetic breast surgery with implants or autologous fat, Dr Maximilian Chlosta aesthetic breast surgery, and the owner, Dr Ludger Meyer-Dobbelstein, aesthetic breast and facial surgery. In Stuttgart, Professor Frank Werdin and Dr Matthias Pfau lead the plastic and aesthetic surgery department at the Diakonie-Klinikum. See also Breast augmentation surgeon in Germany.

The usual German pattern: a consultation with measurements and standardised photographs, a written plan and quote, the reflection period, and follow-up appointments booked in advance. A clinic promising a specific result from an online photo is a warning sign.

Frequently asked questions

How long do breast implants last?

There is no fixed lifespan: implants are not designed to last a lifetime, and the risk of damage grows with age. At regular check-ups, with ultrasound or MRI if needed, you and your doctor decide whether to replace or remove them. Pain, hardening or a change in shape mean coming sooner.

Are smooth or textured implants safer?

Neither is safer overall: each surface has its own risk. Capsular contracture is more frequent with smooth implants; textured ones carry a higher risk of BIA-ALCL, a rare malignant tumour of T lymphocytes (immune cells) that is not breast cancer. With regular follow-up it is usually caught early, when the outlook is good.

When can I fly home after the operation?

When your surgeon clears you, after a check-up and depending on healing. Rushing is unwise: a published case report describes blood collecting around both implants, a haematoma, after a flight one week after surgery. AlenMed therefore books the follow-up visit before the return flight.

How AlenMed organises treatment

After you get in touch, your wishes, photographs and medical information, including any previous breast surgery, are translated and sent to plastic surgery departments. Back comes not a price per cup size but options and an estimate; the plan is only ever made after an in-person examination.

AlenMed has 86 partner hospitals in 38 cities, 345 doctors and its office in Munich. A coordinator handles visa, appointment, translation and accompaniment; the clinic’s invoice is paid directly, and the follow-up visit is booked before the return flight. How to read reviews: see Breast augmentation reviews.

This material is for information only and is not an offer. Whether and how the operation can be done is decided by the doctor at an in-person consultation.
