By number of patients, the largest hospital in Germany is the Charité in Berlin, but people who ask which is the biggest hospital in Germany usually picture a single building, and the Charité is not one. Charité Berlin is Charité – Universitätsmedizin Berlin, the joint medical faculty of the Humboldt University and the Free University of Berlin, working across four campuses: Charité Mitte, Virchow-Klinikum, Benjamin Franklin and Berlin Buch, the last mainly a research site.
What «largest» means depends on the measure: beds, staff, inpatients a year or the number of clinics and institutes, and each measure gives its own list. By its own figures, on 31 December 2025 the Charité had 3,293 beds and 25,256 employees across the group; in 2025 it recorded 147,997 inpatient and day cases and 853,377 outpatient cases in more than 100 clinics and institutes.
The house goes back to 1710, when a plague house was built outside the gates of Berlin and later named Charité. Rudolf Virchow, the founder of cellular pathology, and the surgeon Ferdinand Sauerbruch worked here. Over three centuries it grew into one of the largest hospitals in Europe. What the big university hospitals share is, as a rule, maximum-care status (as the Charité describes itself), nearly every speciality on one site, round-the-clock cover in each and a duty to teach students. What separates them for a foreign patient is the international office: how well it works and whether it exists at all.
What size actually gives a patient
A big hospital in Germany gives a patient experience and back-up. A rare diagnosis is met more often, so it is recognised faster; every subspeciality is on site, so a complication does not mean a transfer; the tumour board meets regularly with all the specialities in the room; and clinical trials run there, which sometimes means access to a treatment not otherwise available.
Size does not give you a personal relationship with the doctor who decides, a short path through the administration or a quiet room. A large house is a machine: you deal with several teams, not one physician.
What matters more than size is the case volume of the one department that will treat you, and its certification: a certified cancer centre or stroke unit, an EndoCert-certified joint replacement centre. German rules work the same way: where, in the view of the Federal Joint Committee (G-BA), the quality of a planned procedure is linked to how often it is performed, a hospital site may offer it only if it expects to reach a yearly minimum, 50 for total knee replacement. A hospital with two thousand beds that does your operation twelve times a year is the wrong address; one with three hundred beds that does it six hundred times is the right one. When a small specialised hospital is the better address
A small specialised hospital is the better address for one well-rehearsed procedure: such houses do one thing all day. Helios ENDO-Klinik Hamburg concentrates on hip and knee replacement and on revision surgery after a failed implant; the German Heart Centre in Munich on cardiac surgery and interventional cardiology; the Herzog Carl Theodor Eye Hospital in Munich on eye surgery, with more than 6,000 cataract and glaucoma operations a year by its own account.
A planned operation often belongs there because the team is fixed, the routine is repeated daily and the ward is built around that one procedure. Paperwork and admission are simpler, and the surgeon is usually the one who examined you.
The line runs where one speciality is not enough: several serious conditions, an unclear diagnosis, or surgery on a patient whose heart or kidneys are already a problem belong in a house with intensive care and every other speciality behind the door.
