Stroke Rehabilitation in Germany: Regaining Movement and Speech

Stroke rehabilitation in Germany is a course of recovery at a neurological clinic for people whose stroke has weakened one side of the body, taken away their speech or disturbed swallowing, and whose families are looking for help to regain what was lost. A stroke is an interruption of the blood supply to part of the brain from a blocked or burst vessel.

The First Hours and Days after a Stroke

In the first days, a person with a stroke is cared for on a stroke unit, a ward where stroke patients are monitored around the clock. A clot blocking a vessel can be dissolved with a medicine given into a vein. A large clot can also be pulled out through a thin catheter passed along the blood vessels. A bleed is treated differently. The sooner treatment begins, the more brain tissue can be saved.

Rehabilitation starts in hospital: once their condition allows, patients are helped to sit up and stand, known as early mobilisation, and their swallowing is checked before they eat or drink. Why Rehabilitation Starts Early

Rehabilitation starts early because the brain can partly reorganise itself. Healthy areas can take over some of the work of damaged ones, which is called neuroplasticity, and regular practice strengthens these new connections. The World Health Organization also notes that rehabilitation should often begin as soon as a health problem is identified. How much function returns cannot be predicted in advance.

What Is Restored: Movement, Speech, Swallowing, Thinking

Rehabilitation works on whatever stands in the way of independent life. A common result of stroke is hemiparesis, weakness of the arm and leg on one side of the body. Physiotherapists retrain standing, balance and walking, and occupational therapists train the arm for everyday tasks such as holding a cup. The weak muscles often become too tense and resist movement, which is called spasticity; stretching, positioning and, where indicated, botulinum toxin injections ease it.

A speech and language therapist works on speech and swallowing. Damage to the language areas can cause aphasia, difficulty understanding or forming language, and the therapist builds exercises around words, sentences and conversation. Safe swallowing matters too: if swallowing is impaired, food and drink can enter the airway and cause pneumonia, so the therapist tests swallowing, adjusts food texture and teaches safe techniques.

A stroke can also affect attention, memory and planning, which a neuropsychologist assesses and trains. Depression after a stroke is common and treatable, so low mood is worth mentioning to the doctor. The family is part of the treatment: relatives learn to help with walking and eating, to talk with someone with aphasia and to continue exercises at home.

Stroke Rehabilitation Centers in Germany: Clinics and Doctors

Stroke rehabilitation is the work of neurological clinics with teams of doctors and therapists. Kliniken Schmieder have six sites in Baden-Württemberg and treat around 14,000 patients a year. Neurorehabilitation at the Allensbach site is headed by Prof. Joachim Liepert, whose focus is stroke rehabilitation with innovative therapy strategies and botulinum toxin treatment; his research concerns the plasticity of the motor system. Prof. Michael Jöbges, medical director of Kliniken Schmieder in Konstanz, works, among other things, on the acute and chronic effects of stroke and researches post-stroke depression.

For swallowing problems, m&i Specialty Clinic Bad Heilbrunn has a neurology department and a centre for swallowing disorders; its head of neurology is Dr Peter Bader. Godeshöhe Neurological Rehabilitation Center in Bonn also treats patients after a stroke, including early rehabilitation. The right clinic depends on the phase, the goals and available places.

Costs and the Estimate

The price depends above all on the phase and the amount of nursing: early rehabilitation with round-the-clock monitoring costs more than a programme for a patient who walks unaided. Length of stay, daily sessions with the speech therapist, neuropsychologist and physiotherapist, room type and a companion’s accommodation also count. Only the clinic can give the exact sum, after seeing the documents.

Frequently asked questions

When can a patient travel to Germany for rehabilitation after a stroke?

A patient can travel once the condition is stable and the clinic’s doctor, having reviewed the discharge report and images, confirms the journey is safe. Walking is not a precondition: early rehabilitation is designed for people who still need a lot of help. After a severe stroke, the transfer is arranged with a medical escort, sometimes by air ambulance.

Is rehabilitation still worthwhile long after a stroke?

Yes, practice can still help months or even years after a stroke: recovery is slower, but training can improve walking, arm use, speech and independence. Neurological clinics in Germany treat the chronic as well as the acute effects of stroke. The doctor judges what is realistic after an examination, without promising results.

Can a relative stay with the patient during rehabilitation?

Yes, accommodation for a companion is arranged in advance: some clinics have rooms for relatives, otherwise a place is found nearby; either way it is a separate item in the estimate. A relative can also join therapy sessions, learn to help with walking, eating and talking, and continue the exercises at home.
