Cataract treatment in Germany usually becomes a question when the world looks veiled, oncoming headlights dazzle and even new glasses barely help. At home the advice was to «wait until it ripens», or an operation has already been offered, and the family wants to know how it is done here.
The only effective treatment is surgery: the clouded lens is replaced with an artificial one, usually as day surgery under local anaesthesia. According to the WHO fact sheet Blindness and vision impairment, cataract and refractive errors are the leading causes of vision impairment and blindness worldwide. Cataract: the signs, and when surgery is due
A cataract is a clouding of the eye’s natural lens, and surgery is due once it starts to get in the way of daily life. It usually comes with age; less often the cause is diabetes, an eye injury or long-term use of steroid medicines (corticosteroids). Congenital cataract in children is a matter for paediatric ophthalmology, where time counts. Typical signs in adults are a veil and blurring, halos and glare from headlights, faded colours, frequent changes of glasses and sometimes double vision in one eye. Drops and tablets cannot make the lens clear again, so treatment is surgical.
The old rule of waiting until a cataract «ripens» no longer applies: surgery is planned when the clouding interferes with reading, driving or work. Waiting without need also has a cost: a very dense lens is harder to remove and the eye harder to measure. With certain other eye conditions, earlier surgery is better; the doctor decides.
Before surgery the eye is measured: biometry records its length and the curvature of the cornea to calculate the lens power, and its accuracy decides whether glasses will be needed afterwards. OCT, a layered scan of the central retina, and an eye pressure check make sure that conditions limiting the result, such as diabetic changes in the retina or glaucoma, are not missed. For travelling patients, tests and surgery are usually scheduled on different days.
How cataract surgery in Germany is performed
Cataract surgery in Germany is, as a rule, phacoemulsification, the standard method. Through a 2–3 mm incision the surgeon breaks up the clouded lens with ultrasound, removes it and places a folded artificial lens in the remaining capsule, where it unfolds by itself; stitches are usually not needed. The eye is numbed with drops or an injection, the patient stays awake, the operation itself takes about 20–30 minutes, and most patients go home the same day.
Some clinics use a femtosecond laser for certain steps, such as the incisions and breaking up the lens. One example is the eye clinic of LMU University Hospital Munich, where laser-assisted cataract surgery is among the fields of the director, Prof. Siegfried Priglinger. The laser is an option, not a precondition of a good result: scalpel and ultrasound are just as safe and effective, and laser surgery costs more.
After surgery, drops follow a strict schedule. In the first days the eye must not be rubbed, a shield protects it at night, and heavy exertion, pools and saunas are off limits; the surgeon sets the time frames. Vision clears gradually, sometimes over several weeks, so the final glasses are prescribed once the eye has healed. A follow-up check is scheduled, and the second eye, if needed, is operated on another day.
Flying home is possible after the follow-up check, once the surgeon agrees, so the return flight is booked around that date in the clinic’s plan. Patients take home the German discharge letter with a translation and the drop schedule for their ophthalmologist. See a doctor at once with increasing pain, a sudden drop in vision or marked redness of the eye: these are signs of rare complications such as inflammation inside the eye. Months or years later the back of the capsule may cloud over (secondary cataract); a short laser procedure treats it without a repeat operation.
Artificial lenses: which one to choose
The choice is between a monofocal lens and lenses that reduce dependence on glasses. A monofocal lens is sharp at one distance: set for distance, it leaves you needing reading glasses, and set for near vision, glasses for distance. It is the basic lens.
A toric lens also corrects astigmatism, an uneven curvature of the cornea that blurs the image. Multifocal and extended-depth-of-focus lenses give sharp vision at several distances and reduce dependence on glasses, but halos around lights at night are possible, contrast can be lower, and they do not suit everyone: with serious eye disease, for example of the retina, they are usually not implanted.
The lens is chosen with the surgeon before the operation, based on biometry, the state of the retina and lifestyle: how much you read, work at a screen or drive after dark. Lenses differ noticeably in price; guide figures are on the page on eye surgery prices.
Where cataract surgery is done in Germany
Both university eye hospitals and specialised eye clinics perform cataract surgery in Germany; the examples below are not a ranking. Heidelberg University Eye Hospital runs a dedicated cataract and lens clinic and a large day-surgery department; its medical director, Prof. Gerd U. Auffarth, specialises in modern cataract surgery and intraocular lenses. How much does cataract surgery cost in Germany?
There is no single price: the clinic states the exact amount in its cost estimate once it has seen the documents. The lens type strongly affects the bill: a monofocal lens costs less than a toric or multifocal one. How much does a German intraocular lens cost?
There is no separate price for a «German lens»: in a German clinic the cost depends on the lens type and model. Monofocal lenses are the least expensive; toric, multifocal and extended-depth-of-focus lenses cost more but correct astigmatism or give sharp vision at several distances. The surgeon chooses the lens based on biometry and the retina, and the clinic lists it separately in its estimate.
Where is the best place to have lens replacement surgery?
Where three things come together: an experienced surgeon, a good choice of lenses and a clinic that follows the patient from measurements to follow-up. Ask how often the surgeon performs this operation and takes on complex cases, which lenses the clinic offers and how it does biometry and OCT. With glaucoma or retinal disease as well, choose a department that handles this regularly. Rankings say little here.
Can cataract surgery be done if you have diabetes?
Yes, diabetes does not rule out surgery, but blood sugar should be well controlled. With high blood sugar and existing diabetic retinopathy, the risk that retinal changes worsen after surgery is higher. So the retina is examined closely beforehand, including OCT; changes found are usually treated first, and after cataract removal the retina is checked again. Its condition also determines how much vision improves.
