Eyelid surgery in Germany: blepharoplasty and double eyelid surgery

Updated

Eyelid surgery in Germany is done by plastic surgeons on the upper or lower lids. People come for two different reasons that look alike from outside: a fold of upper-lid skin that hangs over the lashes and makes the face look tired, and lower lids that stay puffy however much the person sleeps.

This page covers upper and lower blepharoplasty and the operation that creates a crease in an upper lid without one, which many patients know as double eyelid surgery. Other procedures are covered under plastic surgery in Germany.

What eyelid surgery changes and what it does not

Eyelid surgery removes or repositions excess skin, muscle and fat around the eye, nothing more. It does not lift the eyebrow, touch the cheek or remove the lines that come from facial movement.

Two look-alikes need a different operation. A drooping brow pushes the upper-lid skin down onto the lashes; then a brow lift may help more than lid surgery. In true ptosis the muscle that lifts the lid is weak, so the lid edge itself sits low. A mirror test at home cannot tell these apart; an examination can.

When excess skin narrows the field of view, the operation stops being purely aesthetic. A German doctor then measures and documents the visual field before deciding: this is checked, not assumed.

Upper lid, lower lid and double eyelid surgery

Upper blepharoplasty, lower blepharoplasty and double eyelid surgery in Germany are three separate operations. The upper one is a cut in the natural crease that removes a strip of skin and sometimes a little fat; the scar lies in the fold.

Lower blepharoplasty usually targets the fat pads under the eye and the hollow beneath them. The approach may be from inside the lid, leaving no visible scar. The lower lid is the more demanding of the two: removing too much pulls the lid edge down.

In an upper lid without a supratarsal crease, the natural fold above the lashes, double eyelid surgery creates one, either through a full incision or through a few small stitched openings. The choice depends on skin thickness and fat. The crease is planned with the patient in front of a mirror; its height is a matter of proportion and taste, not a standard measurement.

The consultation: what the surgeon checks before agreeing

First come the questions that decide whether to operate at all: dry eyes and how the person tolerates screens and air conditioning, contact lenses, previous eye operations, thyroid disease and allergies.

Then the examination: lid closure, the tear film that keeps the eye moist, lower-lid tone, brow position and photographs in standard views. Medicines are discussed in advance, especially blood thinners and heart medication.

A serious surgeon says no if dry eyes are untreated, the lids do not close well, the cause lies in the brow or the lifting muscle rather than the skin, or the expectations exceed what surgery can do. A second consultation is normal, not a sign of doubt: the operation is planned, so there is time to compare opinions. See the overview of plastic surgery clinics in Germany.

The operation, anaesthesia and the days afterwards

Most eyelid operations are day cases under local anaesthesia, with or without light sedation. General anaesthesia is used when the operation is combined with other procedures, and often for lower-lid surgery, which is more complex, even on its own. The clinic states the duration and the cost in its written estimate.

In the first days the lids are cooled and the head kept raised. Swelling and bruising are worst at the start and then fade. Stitches come out at a check-up; contact lenses and sport wait for a period the surgeon sets.

Before the flight home, plan the follow-up appointment, agree who removes the stitches if the patient leaves earlier, and get the written instructions to take along. Costs are explained on the page about eyelid surgery prices.

Risks, and the limits that are worth knowing in advance

The risks include prolonged swelling, asymmetry between the two sides, dry-eye symptoms that can last weeks and a scar that stays red for months. In the lower lid the edge can be pulled down; this usually settles by itself, and if not, a correction is needed. The patient information of the German Society for Plastic, Reconstructive and Aesthetic Surgery (DGPRÄC) describes the risks in more detail.

The exact look of the lids cannot be promised. A crease behaves differently in thick and thin skin, the remaining swelling settles over months rather than days, and the final look is judged late. In most patients visible swelling goes down within the first weeks, but early photographs say little about the result.

Frequently asked questions

Is double eyelid surgery available in Germany?

Yes, plastic surgeons in Germany perform it. They create a crease in an upper lid that has none, through a full incision or a few small stitched incisions, depending on skin thickness and fat. The crease height is discussed at the consultation in front of a mirror, and the result is judged after several months.

Will eyelid surgery leave visible scars?

There is a scar, but it is placed where it shows little. On the upper lid it lies in the natural crease and is usually hard to see with the eyes open, though it may stay reddish for the first months. Lower-lid surgery done from the inside leaves no visible scar.

When can I fly home after eyelid surgery?

There is no single date: the surgeon decides at the check-up, once healing can be judged. Leaving after the stitches are out is simplest. If the flight is earlier, agree in advance who removes them at home, and take the discharge summary and written instructions. The written estimate states how many days to plan.

How AlenMed arranges eyelid surgery in Germany

AlenMed sends photographs in the required views, the list of medicines, the eye history and any previous operations to a plastic surgeon, who first says what they consider possible. Only then is a written quote issued, followed by the visa invitation letter and an interpreter at the consultation. In Munich, the company works with the Villa Bella clinic for plastic surgery, which offers eyelid lifts.

Ask in advance who will operate, where the scar will lie, what happens if a correction is needed and who pays for it.

This material is for information only and is not an offer; the indication and the method are decided by a doctor at an in-person consultation.
