Blepharoplasty in Germany: Cost and What the Price Includes

Updated

People asking what blepharoplasty costs in Germany want one figure, but a surgeon names it only after an examination. Blepharoplasty is an operation on the upper or lower eyelids: the surgeon removes excess skin and removes or repositions the fat bulges that cause bags.

It is done by plastic surgeons and by eye surgeons specialising in eyelid surgery, a field called oculoplastics. Instead of a price list, this page explains how the price is built and what to check in an estimate; other operations are covered under plastic surgery in Germany.

Upper Lids, Lower Lids or Both: What Changes the Price

Scope changes the price most: upper lids, lower lids or all four. Upper and lower lids can be done in one session, which means more work and a higher estimate.

Technique comes next. On the upper lid the incision follows the natural crease, where the scar later hides. In suitable cases the lower lid is reached from the inside, through the conjunctiva, which leaves no visible scar. Fat is removed or redistributed; surgeons describe repositioning and fixing fat as more laborious, and operating time is billed.

Most eyelid operations are done under local anaesthetic, with a calming injection if needed, as a day case; general anaesthesia or a larger operation may mean a night in hospital. Adding a brow lift or a facelift lengthens the operation and changes the estimate.

Cosmetic or Medical Eyelid Surgery

Eyelid surgery is medical when a drooping upper lid covers part of the field of vision. An ophthalmologist confirms it with a visual field test, called perimetry, done with the forehead muscles relaxed; the criteria are set out in a statement by the German Ophthalmological Society (DOG).

For self-paying patients, doctors bill under the GOÄ, Germany’s official scale of medical fees, in both cases, including services requested beyond medical need, which the bill must label as such. The difference is tax: aesthetic surgery without a medical reason carries VAT, medical treatment does not. The doctor, not the patient, decides whether there is a medical reason.

What the Clinic’s Estimate Includes

An estimate covers the doctors’ work and the clinic’s costs, ideally item by item. Each GOÄ service has a number and points; a point is worth 5.82873 cents, and the result is multiplied by a factor, usually no higher than 2.3 or up to 3.5 with a written justification. There is no blepharoplasty item: the surgeon picks matching items or an equivalent one (§ 6 GOÄ), so the fee shows only in the estimate. Look for:

  • consultation and examination;
  • photo documentation;
  • the surgeon’s fee;
  • an anaesthetist, if sedation or general anaesthesia is used;
  • operating room and materials;
  • histology, if a suspicious growth is removed;
  • follow-up visits and stitch removal.

Flights, accommodation and an interpreter are extra. You stay until the follow-up visit and stitch removal; according to the German Society of Plastic Surgeons (DGPRÄC), stitches come out between the fifth and tenth day, but the surgeon sets the date. Other operations are listed under plastic surgery prices.

Where Blepharoplasty Is Performed in Germany

Both plastic surgery clinics and eye clinics perform blepharoplasty. In Munich, the Villa Bella clinic, a plastic surgery practice clinic, tightens upper and lower lids, the lower ones also by laser, usually as a day case under twilight sedation with local anaesthetic. Its eyelid specialist is its owner and head, Dr Ludger J. M. Meyer-Dobbelstein, a plastic surgeon whose fields include aesthetic breast and facial surgery and reconstructive surgery. The clinic quotes a fixed price covering consultations, follow-ups, materials and anaesthesia.

At the Herzog Carl Theodor Eye Clinic in Munich, eyelids are operated on by Prof. Ulrich C. Schaller, the clinic’s deputy lead physician, who works in a joint practice there; his fields include plastic and reconstructive surgery of the eyelids and orbit and correcting hooded lids and eye bags. PD Dr Konrad R. Koch heads oculoplastics and ophthalmic tumour therapy at the Department of Ophthalmology of University Hospital Cologne. Whether to see a plastic or an oculoplastic surgeon depends on the goal: appearance or lid function.

Frequently asked questions

How much does blepharoplasty cost in Germany?

There is no single price: the clinic gives a figure in a written estimate after an examination. It depends on the scope (upper, lower or all four lids), the technique, the anaesthesia and whether a night in the clinic is needed. Check whether anaesthesia, follow-ups and VAT are included.

Does German health insurance pay for blepharoplasty?

Only for people with German statutory health insurance, and only for a medical reason; purely cosmetic eyelid correction is not covered. If a lid impairs vision, an application with an ophthalmologist’s report goes in before surgery; in doubtful cases the insurer asks the Medical Service (MD) for an assessment but decides itself. Foreign patients without German insurance pay the clinic directly.

Do you pay VAT on blepharoplasty?

Yes, unless there is a medical reason. German VAT law exempts medical treatment (§ 4 No. 14 UStG), and the Federal Fiscal Court treats aesthetic surgery as treatment only when an illness, an injury or a congenital defect requires it, with the therapeutic aim set out in detail. Otherwise 19% is added; the estimate shows whether it applies.

How AlenMed Arranges Blepharoplasty in Germany

For a first assessment, the patient sends front and side photos of the face in natural light, a description of the concern, a list of medicines, especially blood thinners, and an eye doctor’s report if available. Blood thinners raise the risk of bleeding, so the surgeon discusses them beforehand.

A video consultation with a German doctor comes first, then a face-to-face examination, after which the clinic finalises the plan and estimate. The patient pays the clinic directly; AlenMed’s services are charged separately. We prepare visa papers and arrange an interpreter and accommodation near the clinic.

This material is for reference only and is not an offer. The scope of surgery is decided by the doctor at a face-to-face consultation.
