Many women with lipoedema are told for years to eat less and move more, while their legs stay heavy, bruise easily and hurt to the touch. Lipoedema surgery in Germany (lipedema surgery, as spelled outside Britain) is lymph-sparing liposuction, offered when compression and exercise have not relieved the pain; it does not cure the condition.
Lipoedema is a painful disorder of fat distribution, probably inherited, not caused by overeating. Patients come to Germany for its national guideline, staged treatment and surgeons who do this operation routinely. It is part of plastic surgery in Germany, but its purpose is medical.
What lipoedema is, and what it is not
Lipoedema is a symmetrical, disproportionate build-up of fat on the legs and, less often, the arms, sparing feet and hands. It hurts when pressed or touched, may ache on its own, and the legs feel heavy. Many women sense swelling by evening and bruise easily, but neither sign is decisive for diagnosis.
In ordinary obesity, fat is spread evenly, trunk included, and pressure does not hurt. Lymphoedema is swelling from poor lymph drainage, usually asymmetrical and painless under pressure. Each needs different treatment, yet they can overlap: many women with lipoedema are also obese, and obesity can lead to lymphoedema.
Diagnosis rests on history, examination and palpation; no test or scan proves lipoedema, and ultrasound helps rule out other causes of swelling. Stages I to III describe skin and tissue, not pain: the current German guideline (S2k, 2024) advises against using them to rate severity, and stage I can hurt more than stage III.
Conservative treatment comes first
Compression is the backbone: it eases pain and heaviness but removes no fat. Sharp changes in limb size or deep folds call for made-to-measure flat-knit garments, which do not cut in. Exercise in compression and skin care matter; water exercise may help. Manual lymphatic drainage is added if compression falls short, full decongestive therapy if other swelling is present. Surgeons want all this tried first: liposuction is for pain that persists despite it.
Weight loss does not cure lipoedema; in a small series after obesity surgery, leg pain persisted. Yet losing excess weight reduces leg volume, and obesity can worsen lipoedema, so it is treated first. Under German insurance rules, a body mass index above 35 means obesity is treated before liposuction.
The operation: liposuction for lipoedema
Tumescent liposuction is used: fluid with local anaesthetic is infiltrated into the fat, which is drawn out through small openings with blunt cannulas, helped by vibration or a water jet. It spares the lymphatic vessels and stops short of the limit: the aim is less pain and better mobility, not an ideal silhouette.
Legs usually need one to four sessions and arms one or two: at most 10% of body weight comes off at once, so sessions are planned ahead. Legs and arms are treated separately, with several months between sessions, under tumescent local anaesthesia, sometimes with sedation, or general anaesthesia. Day surgery is possible, but German rules require at least 12 hours of observation after removing more than three litres of pure fat in one session.
Garments go on in the operating theatre and are worn almost constantly for several weeks; lymphatic drainage starts at once. Swelling usually settles within three months. Complications are rare: bleeding, wound infection or erysipelas, fluid under the skin, thrombosis, lymphatic damage, uneven contours, loose skin. Liposuction can ease pain for years, but it does not cure lipoedema and no surgeon can guarantee a result. See also rehabilitation in Germany.
Liposuction, laser liposuction and cost
Unlike cosmetic liposuction in Germany, lipoedema surgery splits large volumes across planned sessions, each followed by compression, drainage and follow-up. The lipoedema guideline does not mention lasers, and the insurance rules name only water-jet and vibration techniques; the method is described under laser liposuction in Germany.
Price depends on sessions, volume, anaesthesia, hospital days and garments, so a quote means little without its number of sessions (see liposuction cost in Germany). In 2019 the Federal Joint Committee (G-BA) approved insurance cover only for stage III, temporarily; in July 2025 it decided on regular cover at every stage if, among other things, six months of continuous conservative treatment brought too little relief. International patients pay for themselves, but the indication is accepted.
Choosing the surgeon
Ask how many lipoedema operations the surgeon does a year, whether they work with a lymphologist, how many sessions they plan and what happens if a revision is needed. Under insurance rules, an angiologist, phlebologist, dermatologist or rehabilitation physician makes the diagnosis, not the surgeon. A clinic that fits garments and arranges drainage itself is worth more than a lower quote: both are needed straight after surgery.
The German specialist title in plastic surgery is protected and requires six years of training; «aesthetic» or «cosmetic surgeon» is not, and may mean a doctor for whom liposuction is one service among many. Insurance rules admit plastic surgeons, other surgeons and dermatologists with proven experience. More on choosing a German surgeon.
Frequently asked questions
Can losing weight cure lipoedema?
No. Losing excess weight reduces leg volume, but it does not remove the diseased fat, and the pain may remain. Obesity is therefore treated first and liposuction discussed once weight is stable: under German insurance rules, weight must not have risen in the six months before the decision.
Does surgery help lipoedema?
It reduces pain but does not cure the disease. In the German LIPLEG trial, 410 women in stages I to III were randomly assigned to two groups; over the 12 months after liposuction, pain fell more with surgery than with conservative treatment alone. Data on repeat operations are still pending.
Is compression still needed after liposuction?
At first, yes: garments are worn almost constantly for several weeks, then as advised. How far compression and other treatment can be reduced depends on healing and symptoms: some women need less, some none, others still need it. The doctor following the patient decides.
How AlenMed arranges lipoedema treatment in Germany
AlenMed reviews photographs and reports remotely, chooses a department that treats lipoedema rather than one that simply performs liposuction, and requests a written estimate for the planned sessions. We arrange the visa invitation, admission dates and an interpreter for the consent talk.
Between and after sessions we help with accommodation, fitting and replacing garments, and follow-up visits to the operating surgeon. On discharge the patient receives a letter for her doctor at home.
