
The cost of spine surgery in Germany is set by the hospital in a written estimate and depends on the problem behind it: back pain, a herniated disc, a narrowed spinal canal or a fractured vertebra. Most back pain is treated without an operation, and whether you need one shapes the bill most.

What the Cost of Spine Treatment Is Made Of
The bill combines doctors’ fees, the operation with the hospital stay, and diagnostics in one written estimate, the Kostenvoranschlag, issued in the patient’s name once the MRI images and reports have been reviewed. It covers the surgeon, anaesthesia, days on the ward, materials, implants (screws, cages, an artificial disc) and, if needed, intensive care.
Doctors’ fees outside the hospital rate (consultations, outpatient tests, or treatment by the head of department under a separate agreement) follow the German doctors’ fee schedule GOÄ (Gebührenordnung für Ärzte). Each item has a number and a base rate; a multiplier within set limits reflects difficulty, time and circumstances.
Surgery with a hospital stay is billed per case under the DRG system: one sum for the operating theatre, team, anaesthesia, ward and medication. The operation type, implants and number of levels operated on (a level is a disc with its two neighbouring vertebrae) change the DRG group and the sum.
Spinal MRI and Images Brought from Home
No surgeon plans treatment without a recent MRI, so the hospital first asks for the images themselves, in DICOM format on a disc or via a download link, not just the report. The scan itself is explained on the page MRI in Germany.

A scan is repeated in Germany if the images are several months old and the symptoms have changed, or if the quality is poor. For a fracture or before a fusion, the doctor may add a CT scan or flexion-extension X-rays, taken bending forwards and backwards, to show whether vertebrae shift.
Conservative Treatment and Procedures Without an Incision
Most patients with back pain manage without surgery, at much lower cost. Inpatient multimodal pain therapy lasts 5–7 days: a team treats the pain in several ways at once, with a doctor adjusting medication and giving injections, a physiotherapist working on movement and back muscles, and a psychologist helping with how pain affects sleep and mood.
Injections at a nerve root or the small facet joints, given under CT or X-ray guidance, bring a local anaesthetic and an anti-inflammatory drug to the source of pain. They are short, often without an overnight stay, so the estimate is far smaller than for surgery. Cost of Spine Surgery in Germany: Disc Herniation, Stenosis and Fractures
The cost of back surgery in Germany depends on the approach, and the smallest estimates are for removing a disc herniation or decompressing a narrowed spinal canal. Through a small incision, under a microscope or endoscope, the surgeon removes the herniated disc tissue or widens the canal to free trapped nerves. Operation and stay are short, so the sum is smaller.
Operations that leave implants or material in the spine cost more. A spinal fusion fixes neighbouring vertebrae with screws, and a small spacer, a cage, replaces the disc. A vertebra collapsed by a fracture is reinforced with bone cement, through a needle (vertebroplasty) or after a balloon restores its shape (kyphoplasty). Another option is artificial disc replacement. Implants, and for fusion or disc replacement longer surgery and stay, raise the estimate.
The same operation can cost two patients different amounts. The number of levels, a revision (repeat) operation, other illnesses such as diabetes or heart disease, and a night in intensive care all change the sum. Check whether both estimates include implants, intensive care and follow-up imaging; without them an estimate looks cheaper, but the final bill may be higher.
