Heart surgery in Germany costs €30,000–€40,000 for a coronary bypass and from €5,000 for stenting, while valve and minimally invasive operations are priced by the clinic. Families ask how much it costs and where the figure comes from, and the second matters more: the calculation rests on the statutory DRG system, which prices each hospital case by diagnosis, severity and procedure. So an estimate comes before travel, not a bill afterwards.

Below are guide figures, what an official clinic estimate covers and leaves out, and how to get a calculation for your case. All figures are for orientation; what counts is the clinic’s written estimate.

What the heart surgery cost in Germany depends on

Above all, the cost depends on the approach, then on the patient’s condition and the hospital. Open surgery usually needs a heart-lung machine, a long theatre session and days of intensive care. Catheter procedures such as transcatheter aortic valve implantation (TAVI) or mitral valve repair with a clip (MitraClip) shorten the stay, but the device and instruments are expensive. Second, the patient’s condition: diabetes, kidney failure or widespread atherosclerosis slow recovery, and every extra day in intensive care adds to the estimate, so the calculation allows for deviations from the standard course.

Third, the hospital and the doctor’s seniority. A specialist centre with a hybrid theatre, where surgeons and the catheter team share one room, prices procedures an ordinary hospital cannot offer, and the fees of department heads and senior specialists follow the national GOÄ fee schedule. Valve and repeat operations carry no range: prosthesis prices and the difficulty of access vary too much, and a repeat operation means working through scar tissue. What an official estimate covers

An official estimate (Kostenvoranschlag) is the clinic’s written calculation for the hospital stay, made from the records sent before travel. Patients from abroad usually pay without German insurance, so the sum is deposited with the clinic before admission; if actual costs are lower, the clinic refunds the difference.

The largest block is the operation: the team — surgeons, cardiac anaesthetists, perfusionists running the heart-lung machine, theatre nurses — and consumables: valve prostheses, vascular grafts, stents and oxygenators, which add oxygen to the blood.

Then come ICU and ward days at different daily rates, intensive care being dearer, and the monitoring and medication throughout: coagulation and cardiac marker tests, daily ECGs, follow-up echocardiography, anticoagulants and pain control. If the course departs from the plan, the clinic informs the patient and recalculates the final account on actual costs.

What is billed separately

Flights, a hotel for an accompanying relative, the medical interpreter and airport transfers are billed separately: none of this is in the clinic’s estimate, so budget for it from the start.

Rehabilitation is a separate stage with its own bill, usually charged per day: the daily rate covers accommodation, meals, therapy and medical supervision, so the total depends on the course length. See cardiac rehabilitation in Germany.

Getting a calculation for your own case

To get a calculation, send a recent ECG, the echocardiography report, coronary angiography or CT angiography results, discharge letters and the current medication list. The fuller the package, the smaller the gap between estimate and final account, because the doctors need not allow for what they cannot see.

The estimate and admission date arrive together, and how soon depends mainly on how complete the package is: each missing document adds a round of correspondence. Unstable patients, with worsening chest pain or a recent heart attack, are reviewed first, because the procedure should not wait.

Can I find out the heart surgery cost before travelling to Germany?

Yes. The clinic issues a written estimate (Kostenvoranschlag) before the trip, based on the records you send: ECG, echocardiography report, angiography results, discharge letters and medication list. The guide prices here show the order of costs; only a clinic’s estimate reflects the procedure, your condition and the doctor’s seniority.

Why does the same operation cost different amounts in different hospitals?

Because the doctor, materials and hospital matter as well as the procedure. Senior doctors’ fees follow GOÄ, valve and stent models differ in price, and intensive care stays vary. Even the DRG part varies by region: every hospital in a federal state bills on the same base rate, and each state has its own.

Do international patients pay more than German patients?

Not for general hospital services: in a hospital that bills under the DRG system, the law requires these charges to be the same for every patient. The difference lies elsewhere: patients from abroad pay directly, may choose a senior doctor billed under GOÄ, and pay separately for the interpreter, flights and a relative’s hotel.
