German hospitals do not set prices by eye. Inpatient care is paid through the DRG system (Diagnosis Related Groups): each case is assigned a group carrying a sum fixed under uniform rules. Consultations and imaging are charged under the official German scale of medical fees (GOÄ). An estimate is arithmetic, not bargaining. Partial, Total or Revision Implant: How the Type of Surgery Changes the Price

The type of implant moves the price most. If cartilage has worn away in only one half of the knee, surgeons fit a partial, or unicondylar, implant that replaces just that half and spares your own ligaments: a smaller incision, a shorter stay, a lower bill. When cartilage is gone across the whole joint, a total implant replaces all damaged surfaces; the operation takes longer and the implant is more complex. X-rays and examination decide, not the budget.

Many German centres use computer navigation or robotic systems to help the surgeon set the leg axis precisely and balance the ligaments. DRG pays nothing extra for this technology, so a hospital may list the system and its single-use instruments separately, and the operation then costs more. Implants combine metal parts, usually cobalt-chromium alloy or titanium, with a durable polyethylene insert; for metal allergies there are ceramic- or titanium-coated models. The estimate is drawn up for the model the surgeon chooses.

Revision replaces an implant that has loosened, worn out or become infected. It costs more than a first operation: modular components assembled to fit the bone defect, sometimes bone grafting and more time in theatre. How long implant models last is tracked by the German Arthroplasty Registry (EPRD), which has collected data on hip and knee implants since 2012.

The hospital bill has two parts: tests before surgery, where the main items have guide prices in euros, and the inpatient package, priced only in the hospital’s official estimate. The inpatient package is the main part of the bill: surgical team, anaesthesia, operating room, implant, room, medication and the first physiotherapy sessions. Diabetes, excess weight or heart disease mean extra monitoring and a longer stay, which shows in the estimate.

Rehabilitation After Knee Replacement: Options and Cost

Rehabilitation after a knee replacement is either inpatient or outpatient, and inpatient costs more because its daily rate covers the room, meals, sessions and medical supervision. Orthopaedic rehabilitation starts in hospital on the day of surgery or the day after, when a physiotherapist gets you up and teaches you to walk with crutches. An inpatient course at a specialist centre usually lasts two to three weeks. The outpatient route is cheaper, as you stay in an apartment or hotel, but suits patients who are already fairly fit.

A typical day combines exercise therapy, warm-pool exercises where the water takes weight off the joint, lymphatic drainage against swelling and gait training. The knee is also moved on a CPM machine, a device that gently bends and straightens it while the muscles rest. The cost of a course depends on its length and treatments and is set in the rehabilitation clinic’s estimate.

Who Operates: Hospitals and Joint Replacement Surgeons

Knee replacements are performed by joint replacement surgeons, usually at specialist orthopaedic hospitals and joint replacement centres. Schön Klinik München Harlaching is an orthopaedic hospital with a joint replacement centre. Only then does the hospital assign the DRG group and issue its Kostenvoranschlag.

What is paid separately from the hospital estimate?

Rehabilitation, accommodation outside the hospital, flights and visas for you and a companion are paid separately from the hospital estimate. The rehabilitation centre issues its own estimate.
