Gender reassignment surgery in Germany: what it costs

Updated

People ask about gender reassignment surgery in Germany and its cost hoping for one number, but German clinics have no single item with that name: behind the words stand several different operations, performed in stages and quoted separately.

This page shows which parts make up the total, which steps before surgery also cost money and which expenses almost never appear in the emailed estimate. For German hospital bills in general, see treatment prices in Germany.

Why there is no single price

There is no single price because the set of operations depends on the direction of the transition and on what each person chooses: chest surgery, surgery on the internal reproductive organs, construction of new external genitalia, facial surgery, vocal fold surgery, hair removal.

As a rule, this is not one day in theatre but a sequence of stages months apart, each with its own admission, anaesthesia and invoice.

So a clinic’s honest answer is not a figure from a price list but a written estimate for the plan the surgeon has agreed with this particular patient. Anything else is an approximation that will not match the final bill.

What happens before surgery, and how long it takes

In Germany a long path measured in months comes before surgery: the diagnosis, psychotherapeutic support, an endocrinologist’s assessment and, as a rule, hormone therapy the person already receives. The surgeon asks for documents on all of it. The page on hormone replacement therapy in Germany covers who prescribes it, tests and monitoring.

This is not bureaucracy: part of the change comes from hormone therapy itself, and its result decides how much surgery is needed, for example whether breast surgery is needed. The order of these steps, from diagnosis to the recommendation letter before surgery, is set out in the guideline of the German medical societies.

For the estimate, this means that consultations, monitoring and laboratory tests are paid for too and belong in the total, not in a footnote. Who provides psychotherapeutic support is explained on the page on psychiatry in Germany.

How the estimate for one stage is built

The estimate for one stage is built from the surgeon’s fee under the official German fee schedule, the GOÄ, with a complexity multiplier; the anaesthetist’s fee; theatre and materials; and inpatient days. Under § 5 GOÄ the multiplier runs from 1 to 3.5; above 2.3 it needs particular circumstances and a written justification.

Cost itemDepends onGuide figure
Surgeon’s fee (GOÄ)extent of reconstruction, multiplierper clinic estimate
Anaesthetist’s feelength of anaesthesiaper clinic estimate
Theatre and materialsoperation length, tissue, materialsper clinic estimate
Inpatient daysdays needed after surgeryper clinic estimate
Consultations, monitoring, testsnumber of visits and testsper clinic estimate
Corrective procedureswhether needed, whether includedper clinic estimate

An operation gets longer and the sum larger with the extent of the reconstruction, the tissue used — for example, tissue for the new organs taken from another part of the body — and work in two areas under one anaesthetic.

A line item people rarely ask about is corrective procedures: some results need refinement, and it is better to know in advance how that is charged. Removing the womb and ovaries is a gynaecological operation and work on the urethra a urological one; see urology in Germany and gynaecology in Germany.

What the estimate usually leaves out

The estimate usually leaves out hair removal, voice therapy sessions, facial procedures, consumables and aftercare supplies for home, as well as flights, several weeks of accommodation, an interpreter and a companion.

Nor does it cover the later items: follow-up appointments, endocrinological monitoring and continuing hormone therapy at home. An estimate with none of these lines is not cheaper, only incomplete.

Who pays: the insurance system and a patient from abroad

A patient from abroad pays privately: German insurance shares costs only for people insured in it. Their health insurer may cover part of the cost under conditions set by Federal Social Court (BSG) rulings and the Medical Service (MD) assessment guidance, and only after reviewing the documents. That separate procedure does not extend to visitors.

For a self-payer the sequence is a written estimate, a deposit before admission, the final invoice after discharge and a refund of the unused part. A doctor must give the expected cost in text form beforehand if insurer payment is not secured (§ 630c(3) German Civil Code), and a hospital may request an advance payment without proof of insurance cover (§ 8(7) KHEntgG). Ask before the trip which documents your insurer at home will want; the page on plastic surgery in Germany shows how such estimates are handled.

Frequently asked questions

Can I find out the cost before travelling to Germany?

Yes, as a preliminary figure. From medical reports, the hormone therapy record and test results, the clinic sends a written estimate for the first stage. The surgeon confirms the final plan in person, and if the plan changes, so does the sum, so ask in advance what then happens to the estimate.

Why do different clinics quote such different sums?

Because they usually price different plans. One estimate covers a single stage, another two procedures under one anaesthetic; technique, tissue, inpatient days and the GOÄ multiplier differ too. Compare only estimates for the same plan, and check what each includes and what is billed separately.

How many trips to Germany will I need?

As many as there are stages in the plan agreed with the surgeon. Each stage is a separate trip: consultation, hospital stay and the first weeks of recovery nearby. Stages are usually months apart, so a doctor at home who monitors you and your hormone therapy matters.

How AlenMed arranges this treatment in Germany

AlenMed starts with the documents: reports, the hormone therapy record and monitoring results go to the relevant department. The surgeon says which plan they consider feasible and in what order, and only then do figures appear. Then come a written estimate for the first stage, the visa invitation letter and an interpreter at the consultation and at discharge.

Plan ahead how many weeks each trip takes, where to stay between stages, who monitors you between trips and how your questions reach the surgeon once you are home. We discuss this respectfully and to the point, without judging your decision.

This material is for information only and is not an offer. The indication, extent and order of the stages are decided by a doctor in person.
