A vasectomy in Germany has no single price: the bill consists of several items, and the total depends on the technique, the anaesthesia and how many follow-up tests are needed. A man looking up the cost has usually settled the decision itself and wants one thing: the price, and the certainty that nothing will be added later.
This page covers how the bill is built, what it does and does not include, why a self-payer’s total follows the state fee schedule GOÄ, and why a concrete figure comes only after the consultation. There is deliberately no price list here: any list would be unreliable. For the procedure itself, see Vasectomy in Germany.
What is actually being paid for
You pay for several items, not just the operation: the urologist’s consultation, where he must explain that the procedure is meant to be permanent; an ultrasound of the scrotum if needed; the operation itself; consumables; histology of the removed piece of vas deferens where it is done; and the follow-up semen analysis weeks later.
| Cost item | What it depends on | Guide figure |
|---|---|---|
| Consultation | its length, any ultrasound | clinic’s estimate |
| Operation and consumables | technique, surgeon’s time | clinic’s estimate |
| Anaesthesia | local, or sedation or general with an anaesthetist | clinic’s estimate |
| Histology | whether it is done | clinic’s estimate |
| Semen tests | how many, whether included | clinic’s estimate |
Much of the difference comes from the anaesthesia. The procedure is normally done under local anaesthetic, and the patient goes home the same day; sedation or general anaesthesia adds the anaesthetist’s work and monitoring afterwards, so the gap between «local» and «asleep» is noticeable.
The result is not immediate: only a follow-up semen test confirms it, and the test often has to be repeated. If the clinic charges for these tests separately, ask about it in advance. So «cheaper» and «more expensive» often just mean a different set of included items.
GOÄ: why the bill looks like a list of numbers
Because a German doctor bills a self-paying patient under the state fee schedule, the GOÄ (Gebührenordnung für Ärzte). Every action has a number and a base rate, and the total depends on the multiplier applied: the doctor sets it by difficulty, time and circumstances within the limits of the schedule and needs a written reason to exceed the usual ceiling. The price is not arbitrary: each line shows the date, the service number and name, the multiplier and the amount, so the bill can be checked line by line.
In practice, two clinics can lawfully charge different totals for the same operation by billing different items or multipliers, so compare line items, not «clinic prices»; how treatment costs are calculated is explained on the page Treatment prices in Germany. A written estimate (Kostenvoranschlag) is a normal request, not a sign of distrust: when insurance does not pay, German law already obliges the doctor to state the expected costs in writing before treatment begins (Section 630c of the Civil Code).
Why statutory insurance usually does not pay
Because a vasectomy chosen without a medical indication is, as a rule, something the patient pays for himself in Germany: statutory health insurance covers sterilisation only when an illness requires it. A visitor from abroad is not insured with a German statutory fund, so for him it is self-pay in any case. This is a general rule, not a legal opinion on an individual case.
The bill excludes travel, accommodation, an interpreter and the treatment of any complications: swelling and a haematoma usually settle on their own, but a further visit and examination still cost something. Ask before the operation, not after, what happens if it ever has to be reversed: reversal is microsurgery, harder and dearer than the vasectomy, with no guaranteed result.
Where it is done and what moves the total
Besides technique, the total depends on the type of institution: a urology practice, a specialised urology hospital or a urology department in a large hospital. Partner examples are Urologische Klinik München-Planegg, where Professor Maximilian Kriegmair is chief physician and clinic director; the urology department of ISAR Klinikum in Munich, headed by its director, Professor David Schilling; and the urology department at Agaplesion Markus Krankenhaus in Frankfurt, where Professor Georg Bartsch is chief physician. Their published profile includes uro-oncology and reconstructive urology. The Planegg clinic states on its website that it also performs vasectomies; whether the other two do is checked separately by the coordinator, and the hospital is matched to each request. See also Urology in Germany, Urology clinics in Germany and, for a neighbouring outpatient urology procedure, Circumcision in Germany.
Regional differences matter too: in Berlin the offers and the totals differ from the south of Germany, which is why Berlin has its own page, Cost of a vasectomy in Berlin.
Frequently asked questions
How much does a vasectomy cost in Germany?
There is no single price: the clinic states the amount in a written estimate after the consultation. The total depends on the technique, the anaesthesia, the GOÄ multiplier and the number of semen tests included. When comparing offers, check that all of these are listed: a lower figure may simply mean fewer services.
Can I find out the cost before travelling to Germany?
Provisionally, yes; finally, only after the consultation. In reply to a request describing the situation, the clinic sends an estimate listing the GOÄ items. The total can change if an ultrasound proves necessary or the patient chooses general anaesthesia, so ask for both anaesthesia options in the estimate.
How much does it cost to reverse a vasectomy?
Considerably more than the vasectomy, and again only an estimate gives the figure. Reversal is microsurgery; in the GOÄ it is a separate item with a higher rate, charged for each vas deferens. Success is not guaranteed: the ducts do not always reopen, and not every couple conceives afterwards. That is why a vasectomy is planned as final.
How AlenMed organises treatment
After you get in touch, AlenMed sends the request to urology departments; the reply is a GOÄ estimate listing the items, an appointment date and the length of stay. These figures are the clinic’s estimate, not our price.
AlenMed works with 86 partner hospitals in 38 cities and 345 doctors; the company office is in Munich. A coordinator arranges the visa invitation, the appointment and the interpreter, and makes sure the discharge letter states which procedure was performed and when the follow-up test is due.
This material is for information only and is not an offer. Treatment is decided by the doctor who examines the patient in person.
