Cost of Addiction Treatment in Germany: Detox, Rehab, Estimate

Updated

Families who ask about the average cost of addiction treatment in Germany want a figure in advance, but one average hides the fact that treatment runs in stages of very different length, so the total depends on the stages needed.

This page explains the stages, how German clinics price each of them, what an estimate usually includes and what to add to the budget. The sum itself comes from the clinic once its doctors have reviewed the medical reports. A separate page gives an overview of addiction treatment in Germany.

Why There Is No Single Average Cost

There is no single average cost because treatment is made up of stages that differ greatly in length. Detox and withdrawal take days to weeks, rehabilitation (weaning treatment) takes weeks to months, and then outpatient aftercare follows.

Other factors change the sum too. First the substance: alcohol, opioids, sleeping pills and tranquillisers — benzodiazepine withdrawal is a gradual dose reduction and takes longer. Then complications affecting the liver, heart or nervous system, and co-occurring depression or anxiety. And finally the setting: inpatient, day clinic or outpatient.

Detox in Germany: Qualified Withdrawal and How It Is Priced

Detox in Germany comes in two forms, and the choice shapes the price. Plain detox makes withdrawal symptoms safe, while qualified withdrawal treatment (qualifizierte Entzugsbehandlung) adds psychotherapy and motivational work. In plain words: detox alone clears the alcohol from the body but does not treat the dependence itself, whereas qualified withdrawal helps the person decide on further alcoholism treatment. The German S3 guideline on alcohol-related disorders, in its 2020 version (an update is now being prepared), advises offering qualified withdrawal rather than detox alone and planning it, as a rule, for 21 treatment days. It recommends inpatient care when there is a risk of seizures or delirium, or when abstinence seems out of reach as an outpatient.

Pricing follows the number of days. Inpatient care in psychiatric departments of German hospitals is paid through the PEPP system of daily rates, so the estimate grows with every day spent in hospital. Private clinics quote a daily rate.

For a self-paying patient, doctors’ services are billed under the GOÄ, the official German fee schedule for physicians. Blood tests, ECG and liver ultrasound appear on such a bill as separate lines.

Rehab After Withdrawal: the Longest Part of Treatment

The longest part of treatment is weaning treatment (Entwöhnungsbehandlung), a programme of several weeks that combines group and individual psychotherapy, occupational therapy and relapse prevention. For people insured in Germany it is usually paid by the pension insurance. A patient from abroad pays privately, and for them it is usually the largest item in the budget. How rehabilitation in Germany works is explained on a separate page.

After discharge, outpatient psychotherapy and support groups continue the work. How sessions are charged is covered on the page about psychotherapy costs.

What a Clinic Estimate Includes and What Families Add

A clinic estimate usually includes medical and nursing supervision, withdrawal medication, laboratory tests, room and meals, and therapy sessions. Billed separately are a single room, which is an optional paid service (Wahlleistung), and treatment of other illnesses found during the stay.

On top of the estimate, families should plan for an interpreter: psychotherapy depends on a shared language, so check in advance whether sessions are held in the patient’s language or with an interpreter. Accommodation for a relative, travel and aftercare at home also belong in the budget.

Frequently asked questions

How much does addiction treatment cost in Germany?

The exact sum comes from the clinic’s estimate once its doctors have reviewed the medical reports; there is no single price. The bill combines withdrawal charged by the day, doctors’ services and tests, rehabilitation and extras such as a single room. The total depends most on the length of each stage, the substance and any other illnesses. Travel, a relative’s accommodation and an interpreter are budgeted separately.

Can choosing detox alone keep the cost down?

Detox alone means fewer days on the bill, but the German S3 guideline advises offering qualified withdrawal instead. Detox makes withdrawal symptoms safe but does not treat the dependence itself. Qualified withdrawal adds psychotherapy and motivational work, so it takes longer, and it prepares the person for the next stage, rehabilitation. The choice is made with the clinic’s doctor, depending on the patient’s condition.

Can withdrawal be done as an outpatient or in a day clinic?

Yes, if no severe withdrawal is expected, the person is able to follow the treatment and has support from family or friends; the German S3 guideline allows this. It recommends inpatient care when there is a risk of seizures or delirium, or when abstinence seems out of reach as an outpatient. The doctor chooses the setting after reviewing the reports, and the estimate depends on it: inpatient care includes room and meals.

Will German insurance pay for rehab for a patient from abroad?

As a rule, no. Weaning treatment for people insured in Germany is usually paid by the pension insurance, while a patient from abroad pays privately, which is why rehabilitation is often the largest item in the budget. The clinic states its expected length and setting in the estimate before the trip. Aftercare at home after discharge is budgeted separately.

How AlenMed Organises Addiction Treatment in Germany

AlenMed starts with a confidential enquiry. We need to know the substance, how long it has been used, previous withdrawals and any seizures or delirium, other illnesses and current medication. We translate the documents and send them to a clinic with an addiction department — for example the Department of Addiction Disorders and Consultation Psychiatry at Bürgerhospital Frankfurt, headed by Dr Nina Kim Bekier. Treatment in Germany takes place with the patient’s own consent, which the family should discuss before travelling.

The estimate comes before the trip, payment goes directly to the clinic, and AlenMed’s fee is a separate line. Separate pages explain addiction treatment and arranging treatment in Germany in more detail.

This is reference material and not an offer. The scope and length of treatment are decided by the doctor at a face-to-face consultation.
