Circumcision in Germany

Circumcision in Germany is done for two quite different reasons — a medical problem such as a narrowed foreskin, or a family’s religious and cultural decision — and each path has its own departments, rules and conversation with the doctor. A German clinic will therefore always ask which it is: the answer decides who operates, where and on what terms.

When the Operation Is Medically Indicated

Medically, circumcision in children and adults is done mainly for phimosis, a foreskin that cannot be pulled back over the head of the penis (the glans). In small boys, though, a non-retractable foreskin is normal for years and widens gradually, sometimes until the end of puberty. Paediatric surgeons in Germany therefore do not rush: what counts is complaints and complications, not the narrowing itself. See also Treatment of children in Germany.

Treatment is needed for repeated inflammation of the glans and foreskin, scarring after inflammation, painful urination or ballooning that bothers the child, and recurrent urinary infections in a boy with a significant urinary tract malformation. Paraphimosis, a retracted foreskin trapping the glans, is an emergency.

In children, the usual first step is a steroid ointment; from about the third week the foreskin is gently eased back without pain and always returned, never forced. According to the German guideline, ointment is enough in most cases; surgery follows if it fails, and a foreskin-sparing widening is discussed where possible. The Non-Medical Request and What a Clinic Will Ask

Religious and cultural circumcision of boys is performed in Germany under conditions set out in Section 1631d of the German Civil Code (BGB). Parents may consent to a circumcision that is not medically necessary while their son cannot yet understand and judge it, if it follows the rules of medical practice, but not if, even given its purpose, it would endanger his welfare. Check in advance whether a hospital accepts such requests, and from what age.

Before booking, the clinic asks about the child’s age, who consents and who will attend, general or local anaesthesia, outpatient or inpatient care, and who changes dressings once the family is home. An adult asking about male circumcision in Germany decides for himself and the talk is shorter, but he is still asked about bleeding disorders and blood thinners.

How the Operation Is Organised

Children are operated on by paediatric surgeons and paediatric urologists, adults by urologists — see Children’s hospitals in Germany and Urology clinics in Germany. Whether a department performs this procedure is confirmed on request, not assumed from its name. Examples, not a ranking:

  • Prof. Stuart Hosie, chief physician of paediatric surgery at München Klinik Schwabing; his focuses include paediatric urology and correcting urogenital malformations in children;
  • Prof. Konrad Reinshagen, leading physician of paediatric surgery and paediatric urology at the Altonaer Kinderkrankenhaus in Hamburg, opened in 1859 and now with 237 beds;
  • Prof. Oliver Muensterer, director of paediatric surgery at the Dr. von Hauner Children’s Hospital in Munich; his focuses include minimally invasive paediatric surgery;
  • for adults, Dr. Dimitrios Skambas, medical director and chief of urology at the Asklepios Stadtklinik Bad Wildungen, with an additional qualification in andrology;
  • Prof. Georg Bartsch, chief physician of urology at the AGAPLESION Markus Krankenhaus (Frankfurter Diakonie Kliniken) in Frankfurt; his clinic’s focuses include reconstructive urology.

Children receive general anaesthesia combined, as the German guideline recommends, with a regional block, preferably at the base of the penis; adults usually need only a local anaesthetic. Many go home the same day: the vast majority of paediatric surgery departments in Germany offer outpatient circumcision, though rarely for babies under six months.

Swelling and discomfort last several days; in teenagers and adults pain may return with erections. Dressings are cared for as the clinic shows, sport and swimming wait as long as the surgeon says, and check-ups follow, usually the next day. Healing varies, so the flight home is planned with the surgeon, not against his advice.

What to Prepare and What to Agree in Advance

The clinic needs the child’s medical record or the adult’s summary, earlier urological findings, any family history of bleeding problems, current medication and, after inflammation or scarring, photographs or reports. Agree before travelling:

  • the reason for the request, stated plainly;
  • the type of anaesthesia;
  • outpatient or overnight stay;
  • whether a parent may stay;
  • the language of communication;
  • who removes the dressing, and when;
  • what happens if the surgeon advises against operating for now.

Costs are given in the clinic’s written estimate. This page quotes no prices: they depend on the setting, anaesthesia and the patient’s age, and the estimate is recalculated if the plan changes after examination.

Frequently asked questions

Does phimosis in a child always need an operation?

No. In small boys a non-retractable foreskin is normal and is left alone unless it causes problems such as repeated inflammation, scarring or troublesome ballooning. Treatment usually starts with steroid ointment, which is enough in most cases; surgery follows if it fails.

Can a boy be circumcised in Germany for religious reasons?

Yes. German law lets parents consent to their son’s circumcision without a medical reason if it follows the rules of medical practice and does not endanger his welfare. If a congenital condition such as hypospadias is found, it is corrected first: the foreskin may be needed for the repair.

When can we fly home after circumcision?

There is no fixed rule: the flight date is agreed with the surgeon after a check-up. Leaving before the later check-up needs planning: who removes the dressing at home, which doctor to see for swelling or bleeding, and how to reach the clinic.
