There is no single figure for the cost of breast augmentation in Germany: the bill is built from items, and two clinics may quote different totals for the same operation because their quotes contain different things.
So this page covers what the bill is made of and what to ask, to make comparing offers fair; only the clinic’s written estimate gives the final sum. The operation itself is covered under breast augmentation in Germany.
The items a German bill is built from
The cost of breast augmentation in Germany is made up of the operating team, anaesthesia, the implants and the clinic stay. The first item — surgeon, assistant, theatre nurse and theatre time — is the hardest for patients to check: they see only the total.
Anaesthesia is a separate item with its own chapter in the fee schedule. What breast implants cost in Germany depends on the model, so ask for the manufacturer, size, shape, surface and warranty by name; with lipofilling, the patient’s own fat replaces the implant. BfArM, Germany’s medical devices agency, has reported on the rare lymphoma BIA-ALCL since 2011; most known cases were observed in women with textured implants.
The third group is the clinic and aftercare: a room for a day or a night, dressings, the compression bra, check-ups and histology if needed. Some clinics package these, others bill them separately, so check every line.
| Item | Depends on | Guide figure |
|---|---|---|
| Surgeon’s work | GOÄ items, multiplier, duration | one item, No. 2420: €147.47 at 2.3; total per clinic’s estimate |
| Anaesthesia | type and length | clinic’s estimate |
| Implants or own fat | manufacturer, model, size, surface | clinic’s estimate |
| Clinic and aftercare | nights, what the package includes | clinic’s estimate |
| VAT | medical indication or not | 19% on top if none |
What pushes the figure up or down
The scale of the operation moves the figure most: the incision and the implant’s position, behind the gland or the muscle, are chosen with the surgeon. Lifting the breast as well is a breast lift in Germany, a different operation at a different price, and matching the other side is billed separately.
Revision and implant exchange usually cost more than a first operation: the surgeon works in scar tissue, and with capsular contracture, a hardening of the capsule that the German society of plastic surgeons (DGPRÄC) calls a common complication, the capsule must come out, so the operation takes longer.
The third factor is where you are operated on: a private clinic with a single room or a large hospital department. “Cheaper” sometimes means less is included, not better value: no anaesthesia, say, or no night in the clinic.
How to read a German estimate
Read the estimate against the official GOÄ fee schedule for doctors, which gives each service a number and points. A point is worth 5.82873 cents, and the sum is multiplied by a factor from 1 to 3.5; above 2.3 the doctor must justify it in writing, above 3.5 a written agreement before treatment is needed. Item No. 2420, insertion or exchange of a breast prosthesis, has 1100 points: €147.47 at 2.3, and 25% less for inpatient treatment. Expected costs must be given in writing before treatment (§ 630c BGB).
Ask whether anaesthesia and the implants (which ones) are included, how many check-ups and nights are covered, whether VAT is in, and what happens to the bill if the operation runs long. The final invoice must show each item’s number, amount and multiplier.
Compare lists of items rather than totals, since the same total can hide different contents, and treat a phone quote as committing nobody. Nor should photos decide: German law on health advertising (Heilmittelwerbegesetz) bans advertising cosmetic operations with before-and-after comparisons.
Who pays: insurance and self-payers
The patient pays for cosmetic breast augmentation herself, as German statutory health funds do not cover it. An international patient is a self-payer and pays the clinic directly, by its rules and before the operation.
Reconstruction after cancer surgery, congenital asymmetry or an underdeveloped breast is different: a medical indication may apply, which changes the conversation with the clinic, the insurer at home and the VAT.
What the trip adds to the clinic’s bill
The trip adds flights, accommodation before and after the operation, an interpreter, transport and travel insurance. DGPRÄC puts the usual hospital stay at one to three days, and non-dissolving stitches come out after 10 to 14 days; the surgeon decides at a check-up when you can fly.
Some costs come to mind late: a follow-up visit months later, sending documents and reports, currency exchange and bank charges. See also treatment prices in Germany and plastic surgery prices in Germany.
Frequently asked questions
How much does breast augmentation cost in Germany?
There is no single price: the final sum is in the clinic’s written estimate. The checkable basis is the GOÄ schedule, where item No. 2420 at a multiplier of 2.3 comes to €147.47. That is one line only; other items, anaesthesia, the clinic, implants and VAT come on top.
Are breast implants included in the price?
Only the estimate shows it: implants are a separate item, included in the total or listed on their own line. The fee schedule lets doctors charge materials the patient keeps separately from the fee (§ 10 GOÄ), so look for the manufacturer, model and size.
Do I pay VAT on breast augmentation in Germany?
Yes, if there is no medical indication. Germany’s VAT exemption covers only medical treatment (§ 4 No. 14 of the VAT Act), and a cosmetic operation is not treatment, so 19% is added. After cancer surgery or with a congenital underdevelopment, the indication decides.
How AlenMed obtains the estimate and what happens next
AlenMed sends the patient’s description and photographs to clinics, and the clinic’s written estimate arrives before the trip. We translate it, go through each item with the patient, arrange the visa invitation and provide an interpreter for the consultation.
If the in-person consultation changes the plan, the clinic agrees new items separately and invoices what was actually done. The patient leaves with a discharge report and an implant pass; since 1 July 2024 clinics have had to report each such operation to the German implant register. Follow-up at home is planned with the surgeon before discharge; we translate the reports. See also choosing a breast augmentation surgeon and how to read breast augmentation reviews.
This material is for information only and is not an offer. The final figure is in the clinic’s written estimate; a doctor decides the indication in person.
