Breast Reduction Surgery Cost in Germany: What Shapes It

Updated

Women asking about the cost of breast reduction surgery in Germany want one figure in euros, but a surgeon names it only after an examination. The operation, reduction mammaplasty, removes part of the glandular and fatty tissue and the excess skin, reshapes the breast and moves the nipple and areola higher. More on related operations: plastic surgery in Germany.

This page gives no price list: without an examination, any figure would be unverified. Instead, it explains what makes up the amount and how to get the clinic’s estimate before travelling.

Medical or Cosmetic Breast Reduction

Breast reduction counts as a medical operation when large breasts cause health complaints, and as a cosmetic one when it is done only for shape and size. Typical complaints are neck, shoulder and back pain, bra straps cutting into the shoulders, skin irritation under the breast, and difficulty with sport and finding clothes; the doctor assesses them at the examination. A systematic review of published studies found a statistically significant improvement in neck, shoulder and back pain after the operation.

For the bill, this means that doctors’ fees are calculated under the GOÄ, the German scale of medical fees, in both cases. Medical treatment is exempt from VAT, while purely cosmetic surgery without a medical reason is subject to VAT in Germany, so check how the clinic’s estimate shows it. The doctor, not the patient, decides whether there is a medical indication. The general picture is on our page on plastic surgery prices.

Breast Reduction Surgery Cost in Germany: What Changes the Price

The price depends on the tissue removed, the incision pattern, extra steps, anaesthesia, the clinic stay and histology. The incision runs around the areola, the darker skin around the nipple, and straight down (the “lollipop”), or also along the fold under the breast (the “anchor”, an inverted T). The larger the reduction, the longer the operation. If the breast only needs lifting, see breast lift cost.

Extra steps are counted separately: liposuction at the sides of the breast, correcting asymmetry, or combining the reduction with another operation. Each of them lengthens anaesthesia and changes the estimate.

The operation is done under general anaesthesia, and the surgeon sets the number of nights in the clinic. The removed tissue goes for histology (examination under a microscope), a separate line on the bill.

What the Clinic’s Estimate Includes and What Is Extra

The clinic’s estimate covers the medical part, from the consultation to the follow-up visits, while the trip is paid separately. Check the estimate for these items:

  • consultation and examination;
  • photo documentation;
  • pre-operative tests: blood tests and, depending on age and findings, ultrasound or mammography (a breast X-ray);
  • surgeon’s fee;
  • anaesthetist;
  • operating room and materials;
  • stay in the clinic;
  • histology;
  • compression bra;
  • follow-up visits and stitch removal.

Flights, accommodation and an interpreter are not included. The surgeon says how long to stay in Germany until the follow-up.

Where Breast Reduction Is Performed in Germany

Breast reduction is performed in plastic surgery clinics and departments, for example at the Villa Bella clinic in Munich. Its doctors include Dr. Nadja Zimmer — breast corrections including reduction; Dr. Sebastian Pretzsch — aesthetic breast surgery including reduction and lift; and Dr. Maximilian Chlosta — breast reduction, lift and augmentation. The clinic is led by Dr. Ludger Meyer-Dobbelstein, a full member of the German Society of Plastic, Reconstructive and Aesthetic Surgeons (DGPRÄC).

Diakonie-Klinikum Stuttgart has a department of aesthetic and plastic surgery headed by Prof. Frank Werdin. Choose by the surgeon’s experience with reduction mammaplasty rather than by city, and ask about it at the consultation.

Frequently asked questions

Why is the cost of breast reduction surgery given only after an examination?

Because before the examination the surgeon does not know how much tissue must be removed or which incision is needed. These decide the length of the operation and anaesthesia, the surgeon’s and anaesthetist’s fees and the operating room costs. Extra steps such as liposuction at the sides are also decided at the examination, so the clinic gives its final estimate after it.

Is breast reduction subject to VAT in Germany?

Only if the operation is purely cosmetic and there is no medical indication. In Germany the VAT exemption covers medical treatment, not surgery done only for appearance. The doctor, not the patient, decides at the examination whether there is a medical indication. Check whether the estimate shows VAT as a separate line and which items it covers.

Does liposuction at the sides change the estimate?

Yes: liposuction at the sides is an extra step that lengthens the operation and anaesthesia, which affects the surgeon’s and anaesthetist’s fees and the operating room costs. The same applies to correcting asymmetry or combining the reduction with another operation. The surgeon decides at the examination whether liposuction is needed and includes it in the estimate.

How long should I stay in Germany after the operation?

As long as the surgeon advises: he or she sets the number of nights in the clinic and the date of the follow-up visit. Until that visit you stay in Germany, so plan accommodation near the clinic for the whole period. Flights, accommodation and an interpreter are not part of the clinic’s estimate, so budget for them separately.

How AlenMed Arranges Breast Reduction in Germany

AlenMed starts with the patient’s documents: she sends front and side photos, her height, weight and bra size, a description of her complaints and a list of illnesses and medicines (especially blood thinners), says whether she smokes and adds ultrasound or mammography results if available.

Next comes a video consultation with a German doctor, the clinic’s surgeon, and then a face-to-face examination: only after it does the clinic finalise the plan and estimate. Payment goes directly to the clinic, and AlenMed’s services are charged separately; we help with the visa, an interpreter and accommodation. We make no promises about the result or the size.

This is reference material and not an offer. The scope of surgery is decided by the doctor at a face-to-face consultation.
