Botox price in Germany: per unit, per zone and per year

Updated

The Botox price in Germany is quoted in two incompatible ways: some practices name a figure per zone, others per unit of the drug. Comparing one with the other compares nothing.

This page explains what sits behind each way of quoting, which indications Germany treats as medical rather than cosmetic, and why the sum that matters is per year, not per appointment. See also plastic surgery prices in Germany.

Units or zones: two ways of quoting the same injection

Both sell the same thing: a drug and the doctor who places it in the facial muscles. Botulinum toxin is a prescription medicine measured in units; the forehead, the frown lines and the crow’s feet each take their own number of units, and the same face needs a different dose at different ages.

A price per zone is easier to compare between practices but hides how much drug it contains, so a cheap zone can mean a weak result. A price per unit is honest about the drug if the product is the same, as units of different products are not interchangeable, but says nothing about how many units the doctor will use.

To compare, ask how many units are planned for which muscles, whether the consultation and the follow-up check are included, and what happens if the effect turns out uneven.

Cosmetic or medical: the same drug, a different bill

Botulinum toxin is not only a wrinkle treatment, and its medical use is billed differently. In Germany it is used for chronic migraine, excessive sweating of the armpits and palms, spasticity (stiff muscles) after a stroke, a squint, an overactive bladder and severe teeth grinding; there the dose and injection sites have nothing to do with cosmetics.

For someone insured in Germany, a recognised medical indication is handled differently from a wish to smooth a forehead: statutory insurance may pay for treating a disease, not for improving appearance. A patient from abroad pays for both himself, yet the distinction still decides which specialist he sees: a neurologist, a dermatologist or a plastic surgeon.

This matters for the price: a medical dose is often far larger than a cosmetic one, so the drug, not the doctor’s time, becomes the main line in the bill. More on the field: plastic surgery in Germany.

What a year costs, not what a session costs

Count the year: the effect wears off over a few months and the muscle returns to what it was. This treatment means several appointments a year, so the fair comparison between clinics is the annual figure, including the first consultation and any touch-up.

The annual figure depends on the dose the doctor settles on after the first round, on whether both the brow lines and the crow’s feet are treated, and on how far the patient travels each time; for someone living abroad, that argues for combining the visit with another reason to come. If expectations exceed what an injection can do, consider a facelift in Germany; for the upper face, the surgical alternative is a brow lift in Germany.

What the doctor’s fee is based on

The fee is based on the GOÄ, the official fee schedule for doctors in private treatment, with a multiplier for how demanding the case is: as a rule up to 2.3 for the doctor’s own work, higher only if difficulty or time justify it. The drug is billed separately from the injection, as a medicine.

The invoice can also show the first consultation, photo documentation and a follow-up visit two weeks later to see whether the dose worked. An anaesthetic cream should not appear as a separate line: the GOÄ bars charging the cost of surface anaesthetics.

Ask for a written quote even for a small treatment: only then can you see whether the follow-up is included and what a correction costs. How German bills work in general: treatment prices in Germany.

Who may inject in Germany, and why the cheapest offer is the wrong one

In Germany botulinum toxin is prescribed and injected by a doctor: it is prescription-only, and its label restricts administration to physicians with appropriate qualifications. The decision is medical because the substance is diluted, dosed and placed in named muscles; a misplaced injection shows up as a dropped eyelid or a crooked smile that has to be waited out.

For price comparison: at a cosmetic dose the drug is a small part of the cost and the anatomy the large part, so a figure well below the market usually means fewer units, a shorter consultation or an injector without the relevant training. See also plastic surgery clinics in Germany.

Frequently asked questions

How much does Botox cost in Germany?

There is no single price: the sum depends on the units and zones, the doctor’s GOÄ fee and the drug, billed separately. Per-zone and per-unit figures cannot be compared. The clinic’s written quote shows how many units of which product are planned and what is included.

Does health insurance pay for Botox?

Not for cosmetic injections: German statutory health insurance does not pay for products used to improve appearance. For someone insured in Germany it may cover a recognised medical indication such as chronic migraine. A patient from abroad pays the German clinic himself either way, but the indication still decides the specialist.

How often do the injections need to be repeated?

Usually several times a year, as the effect fades over a few months. The product information advises against repeating an injection sooner than three months after the previous one. The doctor sets the schedule after the first round, and that schedule decides the annual cost.

How AlenMed arranges a botulinum toxin appointment in Germany

AlenMed starts before the trip: the patient says what bothers him and sends photographs. If the reason is medical, earlier findings go to the relevant specialist, who may answer that another treatment fits better. Then come a written quote, the visa invitation letter and an interpreter at the appointment.

Plan ahead how soon after the injection to fly, when the follow-up check falls, what to do if the effect is uneven back home, and how a repeat appointment is scheduled.

This material is for information only and is not an offer. The indication, drug and dose are decided by a doctor in person.
